Before your first visit to RMA we will mail you a New Patient Packet. If you need assistance with the forms please arrive 30 minutes before your appointment time and our office staff can assist you with completion of your paperwork.
To schedule routine follow-up exams, please call two weeks in advance. We request that you arrive 15 minutes before your appointment time.
If you are unable to keep an appointment, please notify us 24 hours in advance. An appointment not canceled 24 hours in advance may be billed as a regular office visit.
- Please contact your pharmacy directly to refill prescriptions
- Allow your pharmacist and Rehabilitation Medicine Associates at least 2 business days to process the refill
- Rehabilitation Medicine Associates will authorize medication refills during the following hours: Office Refill Hours: Mon-Thurs. 9am - 4pm and Friday 9am - Noon.
